Joining the practice takes one text or phone call. Here's what to expect.

- Complete paperwork ahead of timeWe can often help you take care of intake forms before you step foot in the office, so your first visit is about you, not a clipboard.
- Meet Dr. KaoYour first appointment is an unhurried visit to review your history, your goals, and which membership plan fits.
- Choose a planJoin the Foundation Plan at $149/month, ask about the MD Plan for expanded services, or ask about our 6-month Seasonal MD Plan if you spend part of the year in Lakewood Ranch.
Bring with you
- Photo ID
- A list of current medications and supplements
- Recent lab results or records, if you have them
- Insurance card, for lab and specialist referrals
